三、康复护理
1.功能锻炼
原则:伤肢经复位固定后,就可以开始进行相应的功能锻炼。
(1)伤后1~2周:
在医务人员的指导下进行握拳、伸指、腕关节屈伸和旋转运动(图1-2-15)。中段、上段骨折者还可以进行肘关节屈伸运动,但注意不适宜做耸肩及肩关节旋转活动;中段、下段骨折者可进行耸肩及肩关节旋转活动,不适宜做肘关节屈伸运动(图1-2-16)。

(2)伤后3~6周:
上臂中段、上段骨折者可逐渐进行肩关节的耸肩、前后旋肩运动;中段、下段骨折者可逐步进行肘关节活动,健肢扶托患肢上举,至感觉疼痛后缓慢放下回复至原位(图1-2-17)。
(3)受伤7周以后:
以恢复肩、肘关节活动功能为目的,可用健肢带动患肢行外展、后伸、搭肩、后探、磨肩、爬墙等训练。也可逐渐进行抗阻力及负重运动,抗阻力运动时,助手应握住患者伤肢前臂与其进行较轻力量的对抗,至夹板拆除后,可逐渐加大对抗力量(图1-2-18)。

(2)脱:
解开固定带即可脱去支具。
(3)注意事项:
上缘内侧应距离腋窝1~1.5cm;固定松紧以上肢无压迫麻木感为度。
3.居家康复护理
(1)穿、脱衣服方法一:参照第一章第一节肩部骨折穿衣方法。
(2)穿、脱衣服方法二:冬季患肢可不穿入衣袖内,先穿健肢,扣好衣扣,患者手腕从衣缝中穿出(图1-2-20)。
(3)洗漱、如厕时,应使用健侧手进行,以患侧手辅助。
